Oakhaven reaches out into the community through the care given by our Clinical Nurse Specialists (CNS) and our Oakhaven Community Registered Nurses (OCRN) teams. Who work alongside our existing services and provide a more specialised focus on end-of-life care. Working in a patient-centred way and recognising the needs of the patient is at the forefront of what we do – be it medical, emotional or spiritual. The focus of end-of-life care is to ensure the patient, family and caregivers are supported properly at this incredibly an challenging time. Being able to identify these needs is central to what it means to die well. The Oakhaven Community Registered Nursing team started in June 2019 and cover The New Forest, Waterside and Totton six days a week. The team works closely with our Oakhaven Care service as well as GPs, Community Nurses and many other service providers. Helping with pain and symptom control is an essential part of their crisis care and coupled with their hands-on and mindful approach, the team is there to teach, guide and reassure the patient and their loved ones at home. In comparison to GPs we have the luxury of time, meaning we are able to offer conversation and emotional support. We also offer support to families caring
for loved ones at home and teach them the basics of caring for someone right through to giving medication during the night. In addition, the OCRN’s are actively educating care professionals in Residential Care environments. The teaching of palliative care and support they offer is invaluable. When facing a life-limiting diagnosis, most people prefer to remain cared for at home for as long as possible. “Between June 2019 and May 2020 Oakhaven Community Registered Nurses cared for over 200 patients nearing end of life; thanks to their professionalism and compassion nearly all those who preferred to die at home were able to do so.”. “Thank you so much for the care and compassion you provided. Your kindness meant a great deal to me. With your support, she was able to enjoy her time at home and we equally enjoy her company – far more than we ever could imagine.
